Responsibilities
  • Collaborate closely with leadership to shape and implement our customer success strategy from the ground up, ensuring seamless execution.
  • Conduct detailed health-based assessments and develop individualized care plans for patients.
  • Provide comprehensive mental health care to up to 25-30 patients weekly, fostering their well-being.
  • Document all interactions with patients meticulously, ensuring accurate and thorough documentation.
  • Provide and maintain exceptional clinical oversight and ongoing coaching to family caregivers, supporting their growth and effectiveness.
  • Create and conduct engaging group care sessions, fostering a supportive environment for patients.
  • Maintain clinical systems, participate in clinical meetings, and adhere to established procedures, contributing to operational excellence.
  • Build strong relationships by actively participating in community events and resource fairs, expanding our reach and impact.
  • Contribute to marketing efforts aimed at growing our patient panel, leveraging your expertise to attract new clients.
  • Develop educational materials and initiatives to enhance care quality, driving continuous improvement.
  • Serve as a patient advocate, effectively communicating their needs and feedback to our executive team and cross-functional leads, ensuring their voices are heard.
  • Collaborate across different functions to support company-wide initiatives, fostering a cohesive and high-performing team.
  • Utilize technology effectively to maximize your impact and reach, leveraging telehealth software, video communication, and internal communication tools.
  • Meet all background and regulatory requirements, ensuring compliance with relevant laws and regulations.
  • Be authorized to work in the United States and demonstrate flexibility to work in different states, expanding our reach and impact.
Qualifications
  • Active Licensed Clinical Social Worker in Arizona, with a valid Master's level degree and licensure (LCSW) and National Provider Identifier (NPI), maintaining good standing with your licensure.
  • Demonstrated experience working in a healthcare setting and proficiency in utilizing telehealth systems.
  • Sound knowledge of treatment planning, plan of care development, and effective follow-up procedures.
  • Experience with mental health and/or substance abuse diagnosis, enabling comprehensive patient care.
  • Familiarity with geropsychology, family therapy, and cognitive behavioral therapy, ensuring a holistic approach to patient care.
  • Proven experience collaborating with clients to achieve desired goals, fostering positive outcomes.
  • Ability to utilize comprehensive assessments, ensuring accurate evaluations of patient needs.
  • Strong organizational skills with a knack for developing processes and systems, complemented by excellent executional capabilities and meticulous attention to detail.
  • Demonstrated extreme ownership and pride in your work, consistently striving for excellence and delivering high-quality results.

Compensation

Competitive hourly rate ranging from $34 to $38, commensurate with experience and expertise.

Employment

We offer flexible work arrangements, including full-time, part-time, and hourly positions, to accommodate your preferences. Independent contractors are also welcome to apply.
