About the Role This role is for a Registered Nurse (RN) who will plan, organize, and direct home healthcare services. You'll be experienced in nursing, with a focus on community health education and experience. You'll use community resources to plan and deliver services that meet the needs of individuals and families in their own homes and communities.

What You'll Do

  • Patient Care:
    • Conduct initial, comprehensive, and ongoing assessments of patients and their families to identify home health needs.
    • Perform a thorough physical assessment and gather information about current and past illnesses.
    • Deliver professional nursing care using all aspects of the nursing process.
    • Assess and monitor patient status by creating and implementing care plans, regularly reevaluating patient and family/caregiver needs, and adjusting care plans as needed.
    • Use health assessment data to determine nursing diagnoses.
    • Develop care plans with established goals based on nursing diagnoses, incorporating rehabilitative nursing actions, and involving the patient and family in the planning process.
    • Initiate appropriate preventive and rehabilitative nursing procedures.
    • Administer medications and treatments as prescribed by the physician, following the physician's plan of care.
    • Offer guidance and support to patients and families in meeting nursing and related needs.
    • Provide health care instructions to patients as appropriate based on assessments and plans.
    • Assist patients with daily living activities and encourage their efforts towards independence.
    • Serve as a Case Manager, taking responsibility for coordinating patient care for your assigned caseload.
  • Communication:
    • Create, maintain, and submit detailed and relevant clinical notes about the patient's condition and care provided.
    • Document changes and outcomes related to pain and symptom management as appropriate.
    • Communicate with physicians about the patient's needs and report any changes in their condition; obtain and receive physician's orders as needed.
    • Collaborate with community health professionals to coordinate the care plan.
    • Educate patients and families/caregivers on self-care techniques as appropriate.
    • Provide medication, dietary, and other instructions as ordered by the physician.
    • Identify and leverage opportunities for health counseling with patients and families/caregivers.
    • Work effectively with the interdisciplinary team.
    • Maintain a safe environment for the patient.
    • Support the patient, family/caregiver, and other team members in ensuring continuity of care.
    • Collaborate with the family/caregiver and Home Health care team to address the emotional and physical needs of the patient and family/caregiver.
    • Attend Case Conference meetings.
  • Additional Responsibilities:
    • Participate in on-call duties as outlined in the on-call policy.
    • Ensure that equipment, items, and services are readily available.
    • Supervise ancillary personnel and delegate tasks when needed.
    • Take responsibility for your professional growth and development, maintaining and enhancing your knowledge and skills through continuing education and in-service training.
    • Fulfill assigned case responsibilities.
    • Actively participate in quality assessment and performance improvement teams and activities.
    • Carry out other duties as assigned.

What We're Looking For

  • Education: Graduate of a National League for Nursing accredited school of nursing.
  • Experience: Minimum of two years of nursing experience, with at least one year in public health, home care, or home health nursing preferred.
  • Licensure: Registered nurse with a current license to practice professional nursing in the state.
  • Skills: Demonstrates excellent observation, problem-solving, verbal, and written communication skills, as well as nursing skills according to competency checklists.
  • Other:
    • Ability to organize and prioritize workload independently.
    • Physical ability to walk, stand, lift, position, and/or transfer patients, and lift supplies and equipment.
    • Ability to reach, stoop, bend, kneel, and/or crouch.
    • Good vision and hearing to effectively perform nursing tasks.
    • Current CPR certification.
    • Valid driver's license, auto insurance, and access to a vehicle for agency-related travel.
    • Self-directed and able to work with minimal supervision.

Compensation

  • Salary: $40-45 per hour
