The RoleΒ πŸ§‘β€πŸ’Ό

As a Social Worker, you'll be a key member of our care delivery team, providing psychosocial support and interventions to patients and their families. You'll play a vital role in enhancing patients' coping mechanisms, coordinating care transitions, and educating patients and their loved ones about psychosocial factors related to illness.

What You'll DoΒ πŸ“

  • Conduct and document comprehensive psychosocial assessments, interventions, and care coordination.
  • Offer crisis intervention and counseling to help patients and their families navigate challenging situations.
  • Collaborate with the interdisciplinary team and patients/families to plan for discharge and identify appropriate post-acute resources.
  • Educate patients, families, and healthcare professionals about psychosocial aspects of illness.
  • Actively participate in initiatives to expand the role of social work within the organization.
  • Other duties as assigned.

What We're Looking ForΒ πŸ”Ž

Required:

  • Master's Degree in Social Work
  • 5+ yearsΒ of post-graduate social work experience in healthcare settings (inpatient, outpatient, home health, etc.)
  • 2+ yearsΒ of experience in case management-related activities.
  • Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW)

  • Strong understanding of cultural competency in patient care.
  • Knowledge of the link between lifestyle and disease processes.
  • Proficient in Microsoft Office Suite.
  • Collaborative team player with excellent communication skills.
  • Ability to handle multiple priorities and work independently.
  • Strong analytical and decision-making capabilities.
  • Experience with motivational interviewing and advance care planning.
  • Adaptable and thrives in a fast-paced environment.
  • Exceptional organizational and time management skills.
  • Competent in using various computer software, including Microsoft Office products.

Additional Information ℹ️

  • Work Schedule:Β Monday-Friday, no evenings or weekends.
  • Location:Β Indiana County, PA and surrounding areas.
  • Travel:Β 50% - 75%
  • Pay Range:Β $57,700 - $106,700Β (Base pay is determined by experience and qualifications).
