๐งโโ๏ธย The Roleย The Lead Patient Care Coordinator will be a key part of the team, helping patients with their mental health journey. This role involves overseeing other patient care coordinators, ensuring top-notch patient care, and providing support to the team.
Responsibilities
- Oversee patient care coordinators to ensure procedures are followed.
- Travel to facilities in Austin (and potentially Dallas & Houston for training).
- Collaborate with the Practice Manager and Director.
- Conduct weekly check-ins with care coordinators.
- Manage care coordination for TMS, Esketamine, and Ketamine.
- Provide quarterly trainings to the care coordination team.
- Manage, schedule, and assign consultations.
- Conduct initial patient consultations and document findings.
- Educate patients about their treatment options.
- Facilitate weekly check-ins with clinicians/providers.
- Complete TMS Certification (training provided).
- Maintain accurate and descriptive medical and clinical documentation.
- Demonstrate knowledge of medical records, authorizations, and lab requisitions.
- Implement consultative interventions.
- Obtain and document patient insurance information.
- Answer phones professionally and relay messages.
- Ensure patient treatment is rendered within approved authorizations.
- Provide excellent and compassionate patient care.
- Serve as a liaison between care providers and patients.
- Use an Electronic Health Record.
- Build trust with individuals to improve patient care.
- Communicate plans of care to insurance case managers and healthcare partners.
- Refer clients to TMS treatments based on eligibility.
- Discuss financial obligations and insurance coverage with clients.
- Protect patient confidentiality.
- Accept new requests and explore opportunities to improve work.
- Perform other duties as assigned.
- Willingness to travel between facilities as needed.
Qualifications
- Associate's degree required, Bachelor's degree preferred.
- 5+ years of TMS and/or Mental health experience.
- Excellent customer service skills (7+ years minimum).
